Notice of Privacy Practices

PURPOSE

To ensure patients are informed about how their protected health information (PHI) may be used and disclosed by Lexington Radiology Associates and how they can access this information.

POLICY

Lexington Radiology Associates will offer a Notice of Privacy Practices to all patients and obtain acknowledgment of receipt. The Notice will be prominently displayed in the reception area and on our website.

PROCEDURE

1. Content of Notice - the Notice of Privacy Practices will include:

• How Lexington Radiology Associates may use and disclose PHI.
• Patient rights regarding their PHI.
• Lexington Radiology Associates 's legal duties with respect to PHI.
• Contact information for questions or complaints.

2. Distribution of Notice

• Offer the Notice to all new patients at their first appointment.
• Make the Notice available upon request.
• Post the Notice in a clear and prominent location in facility.
• Publish the Notice on Lexington Radiology Associates's website

3. Acknowledgment of Receipt

• Obtain written acknowledgment that the patient received the Notice.
• If acknowledgment cannot be obtained, document good faith efforts and reason for failure (e.g., patient refused).

4. Revisions to Notice

• Promptly revise the Notice when there is a material change to uses or disclosures, patient rights, legal duties, or other privacy practices.
• Make the revised Notice available upon request and on the website.
• Post the revised Notice in the reception area.

5. Workforce Members Training

• Train all workforce members on the contents of the Notice and procedures for distribution.
• Ensure workforce members can answer basic questions about the Notice and direct patients to the Privacy Officer for detailed inquiries.

6. Complaints

• Include information in the Notice on how patients can file complaints about privacy practices.
• Direct all complaints to the Privacy Officer for investigation and resolution.

POLICY REVIEW

Lexington Radiology Associates will review our Notice of Privacy Practices policy and procedures periodically, and make updates, when necessary, to ensure they are up to date.
